IV Wellness Lemont Membership Terms and Conditions
Cancellation Policy
Members may cancel their membership by calling or texting IV Wellness Lemont at 630-296-4416 during business hours.All memberships require a minimum three (3) month commitment from the date of enrollment.Cancellation requests must be submitted at least 24 hours prior to the next scheduled membership charge to prevent the next billing cycle from being processed.If a membership is canceled before completing the three-month minimum commitment, a $100 early termination fee will be charged to the card on file.Upon cancellation after the minimum commitment has been satisfied, all membership benefits, including any accrued credits, will remain active until the end of the current billing cycle. After this period, membership benefits and credits will expire.
Credit Roll-Over and Usage
Unused monthly infusion credits roll over to the next month and may be used within 12 months of accrual.Credits may be used while membership is active or during an approved membership pause, provided they remain within their validity period.Credits have no cash value and are non-transferable unless otherwise specified.
Membership Pause
Members may request to pause their membership after completing the initial three-month commitment period.Pause requests must be communicated to IV Wellness Lemont before the next billing cycle to prevent additional charges.During a pause:• No monthly membership charges will occur • No new credits will accrue • Previously accumulated credits may still be used during the pause period (if within their 12-month validity window)If a member specifies a pause duration (e.g., three months), the membership will automatically resume once this period ends and monthly billing will restart at the standard membership rate.Members may request early reactivation of their membership at any time by contacting IV Wellness Lemont.
Refund Policy
All membership fees and service purchases are non-refundable. Members are encouraged to utilize their membership benefits to receive the full value of the program.By enrolling in a membership, members authorize IV Wellness Lemont to charge the card on file for recurring membership fees and any applicable early termination fees in accordance with the membership terms.
Birthday Injection Eligibility
The complimentary birthday injection benefit is available to members starting after the first full month of active membership.
Transferability of Benefits
Membership benefits are generally non-transferable and may only be used by the registered member.Diamond memberships may be shared with one designated family member.
Service Availability and Scheduling
All services are subject to availability and may require advance scheduling.
Changes to Membership Benefits and Pricing
IV Wellness Lemont reserves the right to change membership benefits and pricing at any time without prior notice.
Age Requirement
Members must be 18 years or older to join.
Dispute Resolution
In the event of a dispute, members agree to first contact IV Wellness Lemont to seek a resolution. If the issue cannot be resolved, both parties agree to resolve the dispute through binding arbitration in the jurisdiction of Lemont, Illinois.
Limitation of Liability
IV Wellness Lemont is not liable for any incidental, indirect, special, or consequential damages arising out of or in connection with the membership services, except as required by law.
