IV Wellness Lemont Membership Terms and Conditions
Cancellation Policy
Members may cancel their membership by calling or texting IV Wellness Lemont at 630-296-4416 during business hours. To ensure a fair usage of our services, we require members to maintain their membership for a minimum of two months before eligibility for cancellation. Cancellation requests must be submitted at least 24 hours prior to the next scheduled membership charge following this two-month period to process the cancellation effectively. Upon cancellation, all membership benefits, including any accrued credits, will remain active until the end of the current billing cycle. After this period, all benefits and accrued credits will cease.
Credit Roll-Over and Usage
Unused monthly credits roll over to the next month and can be used within 12 months of accrual with an active membership.
Membership Pause
Members have the option to pause their membership. During the pause period, members will not have access to any membership benefits, including the use of accumulated credits. All benefits will remain inactive until the membership is resumed. Requests to pause a membership must be communicated to IV Wellness Lemont by the last day of the current paid period to ensure the pause takes effect for the upcoming month. While the membership is paused, it will be considered dormant, and no charges will be incurred.If a member specifies a pause duration (e.g., 3 months), the membership will automatically resume once this period elapses, and the member will be charged the standard membership fee for the upcoming month, reinstating their access to all membership benefits, including previously accumulated credits (if within their validity period). If a member wishes to resume their membership earlier than the specified pause period, they must inform IV Wellness Lemont to facilitate this change.
Refund Policy
All membership fees are non-refundable. Members are encouraged to utilize their membership benefits to their fullest extent.
Birthday Injection Eligibility
The complimentary birthday injection benefit is available to members starting after the first full month of active membership.
Transferability of Benefits
Membership benefits are non-transferable and can only be used by the registered member.
Service Availability and Scheduling
All services are subject to availability and may require advance scheduling.
Changes to Membership Benefits and Pricing
IV Wellness Lemont reserves the right to change membership benefits and pricing at any time without prior notice.
Age Requirement
Members must be 18 years or older to join.
Dispute Resolution
In the event of a dispute, members agree to first contact IV Wellness Lemont to seek a resolution. If the issue cannot be resolved, both parties agree to resolve the dispute through binding arbitration in the jurisdiction of Lemont, Illinois.
Limitation of Liability
IV Wellness Lemont is not liable for any incidental, indirect, special, or consequential damages arising out of or in connection with the membership services, except as required by law.
